PHA has developed this webpage to provide home-based care providers with distribution plans affecting them, including county and municipality distribution plans, as well as opportunities to receive a COVID-19 vaccine through an approved vaccine provider. Allegheny The Allegheny Health Department is gathering information from organizations with healthcare personnel who fall into the Phase 1A category for vaccinations. These organizations must provide basic contact information and details on operations, as well as an estimate of eligible personnel working in Allegheny County. One point of contact for each organization is asked to complete a COVID Vaccination Request form through the county’s Online Support Center. You will need to create an account and select the options for Request Service, Health Department and, finally, COVID Vaccination Request, to submit the information for your organization. BethlehemThe Bethlehem Health Bureau is coordinating vaccination efforts for healthcare workers with the Allentown Health Bureau, Lehigh Valley Health Network and St. Luke’s University Health Network. Walgreens and CVS additionally will begin vaccinating residents and employees in long-term care facilities this week. Agencies interested in receiving the Moderna vaccine that are part of the 1A priority group are encouraged to register by going to the City of Bethlehem’s COVID-19 webpage and clicking under the “vaccines tab” or completing this survey. The Bethlehem Health Bureau expects to receive vaccine shipments on a weekly basis. BucksSt. Mary Medical Center in Langhorne Unaffiliated healthcare personnel can request COVID-19 Vaccine at the St. Mary Medical Center Vaccine Clinic here. Once the form is completed and proof of occupation submitted, a team will review the applicant's eligibility to receive the vaccine based on the Phase 1A sub-prioritization guidance in Pennsylvania's COVID-19 Interim Vaccination Plan. If an applicant is determined to be eligible, they will be called to schedule vaccine appointments. Requestors should expect a response within two (2) business days. Bucks County Health Department Unaffiliated healthcare personnel should complete this survey. The practice/individual must operate and/or live in Bucks County, PA. Email the completed survey to [email protected]. When BCDH Vaccination staff has the capacity to vaccinate your staff, we will contact the named Point of Contact. In the interim, healthcare personnel should attempt to obtain their vaccination from other sources (e.g., area hospitals which will have an allotment of vaccine for unaffiliated providers, and each individual should contact their own primary care provider). Chester and DelawareChester and Delaware counties are joining with healthcare organizations in both counties in vaccinating the first two groups within Phase 1 - frontline healthcare personnel and EMS providers. This week, Chester county sent a link to home-based care providers to register employees. The consent form (found within the registration confirmation email) must be completed and printed before arrival at the vaccination site. Vaccination will not be conducted without a consent form. Chester (and Montgomery) County has reached out directly to home-based care providers. If providers have not heard from a county representative, they should contact the county health department for more guidance. Fayette, Greene and WashingtonCenterville Clinics, Inc. has been approved as a provider for the COVID-19 vaccine and is willing to help with vaccine administration to eligible individuals who reside in Washington, Fayette, and Greene Counties that fall into Category 1A as outlined in the Pennsylvania Covid-19 Interim Vaccination Plan. Vaccines will be administered at the Washington, Centerville, and Uniontown locations. If you are interested in utilizing Centerville Clinics for your vaccine distribution, please complete this survey. You must complete the survey to begin the process; upon completion of the survey, Centerville Clinics will reach out to you directly to plan vaccinating your eligible personnel. As part of this planning process, all providers must complete a roster of all individuals who wish to be vaccinated; the roster must include all requested information such as name, age, position, allergies, COVID-19 diagnosis and other information necessary to determine eligibility. LehighThe Allentown Health Bureau is now scheduling appointments by email only for the Moderna COVID-19 vaccine for qualifying healthcare staff in Lehigh County. The Bureau has been forced to switch from phone call appointment scheduling requests to email requests because of extremely high call volume. Until further notice, appointments are no longer being scheduled at 610.437.7760. The City has begun direct outreach to providers and has begun hosting a series of vaccine clinics to inculcate approximately 15,000 home health providers who are not associated with an approved vaccine distributor. Jefferson Health Jefferson Health is allocating 10% of the COVID-19 vaccine supply it receives in Montgomery and Bucks Counties to certain community healthcare workers. At present, this limited vaccine supply is available only to individuals who live or work in Montgomery County or Bucks County and who work in a paid or unpaid capacity in a health care setting, and in a role that carries the potential for direct or indirect exposure to COVID-19 patients or COVID-19 infectious materials. Lower Bucks Hospital Submit your request by contacting [email protected] or by calling (215) 785-9419. Anyone requesting a vaccine can expect a response within two (2) business days. If it is determined that the applicant is eligible per State guidance and as supply allows, they will receive a call to be scheduled for both their first and second vaccine appointments. Lower Bucks Hospital only carries the Pfizer-BioNTech COVID-19 Vaccine and second appointments will be scheduled for 21 days after the first dose. Prior to the appointment date detailed instructions will be provided on what to bring, how to enter the facility, how to register on-site, and where to find the vaccine clinic. Vaccine Appointment Day: On the day of your appointment please arrive 15 minutes ahead of your scheduled time slot. All recipients must bring their government-issued identification (Driver’s License Preferred) to the appointment. All vaccine recipients will be required to stay for a 15-minute observation after receiving each dose of the vaccine. Note: Recipients will either receive the Moderna or Pfizer vaccine, depending upon availability. Both vaccines require a second shot or booster. For the Pfizer vaccine, the booster should be administered within 19-23 days of the first shot. For the Moderna vaccine, the booster should be given within 26-30 days. You will be able to schedule your booster appointment at the time of your first vaccine appointment. Your booster appointment will be at the same location as your first appointment.
