The first Home Health Value Based Purchasing program performance year has concluded, and home health agencies will see payment adjustments in 2025 based on their performance in the past year. Now is a great time to look back on 2023 to identify lessons that have been learned so strategies can be developed for 2024. March 25 – Updates to HHVBP, Lessons Learned, CMS Reports
This webinar will review the HHVBP model and metrics included in the Total Performance Score, along with the changes coming in 2025. We will discuss trends that have been identified based on reports released from CMS and what they may mean for your agency. Lastly, we will outline how to determine which measures to focus on as you develop a plan for improvement in 2024.

  • Recall lessons learned from the first HHVBP performance year.
  • Understand how to prioritize metrics for improvement.

April 8 – Patient Satisfaction Measures
This webinar will review the Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS), which are designed to measure the patient’s experience of receiving home health service. We will address strategies centering the patient in the delivery of care to enhance their experience. We review the specific questions impacting HHVBP scores in this measure, and steps that can be taken to improve in these measures. Lastly, we will discuss prohibited actions in relation to the HHCAHPS.

April 22 – Claims Based Measures

This webinar will review the claims-based measures included in 2024 and discuss the new claims-based measure changes coming in 2025. Developing hospitalization prevention strategies today can position an agency for success this year and into 2025. Appropriate discharge planning is important now and will continue to be as we get closer to 2025. This webinar will discuss tools to succeed in reducing hospitalizations and improving discharge planning.

  • Understand the conditions included in the Potentially Preventable Hospitalization metric coming in 2025.
  • Identify approaches to improve discharge planning.

May 6 – OASIS Functional Scoring Measures
This webinar will indicate the specific OASIS items that contribute to HHVBP measures in 2024 and what will change in 2025. General OASIS guidance, exclusions, item specific guidance, and how the DC Function Score is calculated will also be reviewed.

  • List the OASIS items that will not change in 2025.
  • Identify the items that will be included in the DC Function Score.
  • Recall general OASIS strategies to improve accuracy.
